We are SO excited to have your student participate in this one-of-a-kind, hands-on experience!
BUT FIRST.....
For everyone's safety and protection, any student who wishes to participate in any FTC Adventure or Retreat MUST have a guardian complete this permission form prior to the activity. A student will not be allowed to participate if he or she does not have a guardian fill out and electronically sign the permission form. Please contact FtC with any questions or concerns.
